How do I transfer my record?
	Transferring from another practice, including record release form for incoming records If you are transferring to Premiere Pediatrics of Harnett Health from another practice, we appreciate your confidence in having us provide your child’s healthcare.  To help you request a transfer of your records from your current pediatrician, we have provided below a Record Release Form for Incoming Records.  You can fill out the form by hand or electronically while you are online.  Then print the form, be sure to sign it, and send it to your previous physician’s office. Viewing the form requires Adobe Acrobat Reader (link), available for free from the link.  If you are using an older version of Adobe Acrobat Reader, you may get an error message.  If this is the case, please download and install the latest version. When you come for your first visit with our practice, make sure you bring with you the (insert forms that you want the patient to bring – Family History, Medical Questionnaire, etc.  You can put links for these forms on the left hand side of the page).     Record Release Form – Incoming Records Transferring out of the practice, including record release form for outgoing records We are sorry that you are transferring out of your practice, but we understand that you have reasons for making a change.  To help you request a transfer of your records to another office, we have provided below a Record Release Form for Outgoing Records.  You can fill out the form by hand or electronically while you are online.  Then print the form.  Be sure to sign it and tell us the reason for your transfer.  Either mail or bring the signed form to our office so we can transfer your records.
