The School Systems in our area require all children to receive a complete physical that includes weight, height, blood pressure, vision screening and updating immunizations.

All counties provides this form. Please bring the form with you to the office visit so we can fill it out after the exam and return it to your school. Forms are also available through your individual schools. Most elementary schools offer pre-registration, normally in the spring, before you child starts school. Please call early to schedule your appointment for a kindergarten physical.
