Local school systems may require that children who try out for school sports have an annual exam. Daycare providers may also require physical exams annually or every two years. Even though we may see your child regularly during the year for other reasons, these visits are not considered as sports or daycare exams, and we simply cannot use them regular or sick-child visits in the past to complete your child’s form. Please call to schedule your sports, camp, and daycare physicals as soon as you know that you need them.
